Executive overview

From fragmented services to family-defined, measurable mobility.

MATCH Family Success Infrastructure is a developing, replicable coordination model designed to help communities organize systems, relationships, and opportunity pathways around one family journey.

The honest truth

Families do not experience life in silos. Systems often do.

Housing, education, health, behavioral health, employment, courts, and child welfare may each carry a portion of a family’s story. Too often, the family must carry the connections between them.

The Navigation Gap
Multiple plans, disconnected referrals, repeated stories, unclear ownership, and too little recovery when a connection fails.

The MATCH response
One family-defined plan, trusted navigation, accountable partners, mobility pathways, and visible learning.

The practical aim
Help communities turn what they already have into a more coherent family journey, rather than adding another isolated program.

How the MATCH model works

Five disciplines. One connected operating system.

M

Map

See the person, family, system, assets, barriers, relationships, and opportunities before prescribing action.

A

Align

Put people, organizations, resources, and accountability around one shared journey and a clear destination.

T

Technology

Strengthen visibility, coordination, personalization, and learning without replacing trusted human relationships.

C

Coach

Activate Navigators, mentors, families, leaders, and trusted adults to turn plans into movement.

H

Harvest

Measure movement, learn from implementation, improve the system, and scale only what evidence can support.

The proposed two-county strategy

Test the core in two different places before claiming a scalable solution.

Founding demonstration context

Franklin County

Proposed founding urban demonstration context, designed to establish the core operating standard, local partnerships, learning methods, and implementation evidence.

Selection in progress

Second county

Intended rural or smaller-county replication environment, selected through readiness criteria to test the model across a different service and access landscape.

This overview does not state that implementation agreements are executed, that a statewide system is complete, or that results are already verified.

What the county unit includes

Readiness

Local mapping, family and community context, partner capacity, and navigation friction assessment.

System design

Design Labs, governance, shared workflows, consent boundaries, and accountability.

Family navigation

Trained relationship partners, Family Success Plans, warm handoffs, and follow-through.

Partner capacity

Community, faith, mentor, school, provider, and employer readiness and support.

Mobility pathways

Stability, education, credentials, work, housing, financial capability, and relationships.

Evaluation

Family experience, implementation learning, outcome movement, and replication evidence.

What MATCH will measure

Outcomes that hold the whole journey in view.

Public reporting will focus on family stability, youth stability, economic mobility, system efficiency, and public value. Measures, baselines, and results will be published only as the evaluation framework and local approvals mature.

What MATCH will protect

Privacy, choice, and the difference between a public model and a case-management system.

The public website does not collect case records, deliver clinical care, make statutory decisions, or show protected family information. Those boundaries are part of the infrastructure.

The invitation

Explore what Family Success Infrastructure could look like in your community.

MATCH invites communities, families, public systems, community organizations, funders, workforce partners, and researchers to design, test, and strengthen a different way forward.
